This is a new design of our client consultation forms x 50.

The consultation form now consist of 2 parts: the electronic version and the printed version. 

The electronic version

Contains everything the client needs to know before attending their appointment, what to expect during during and steps to take after. Suitable for all facial areas - eyebrows, lips and eyeliner. Electronic form has an area for you to add your salon details and by booking an appointment with you, the client automatically agrees to all mentioned in electronic version. 

The printed version

At the time of the appointment client needs to only fill out the second part of the consultation form (the printed version that you then file). Creating the forms this way has helped us to save time your client spends before appointment and also space by storing less printed versions on site.
